What is National Digital Health Scheme Mission OR PM Modi Health ID card Yojana

Our Honorable Prime Minister Narendra Modi on the eve of 74th Independence Day Celebrations, announced the “National Digital Health Scheme Mission,” also termed as “PM Modi Health ID card Yojana.” The National Digital Health Mission comes under Aayushmaan Bharat run by the Central Government of India collaborating with the Health Ministry. The objective of the National Digital Health Mission is to integrate all the health services by developing an advanced medical infrastructure and to combine it into a Single Health Care Eco System. This Digital Health ID Cards System helps provide efficient and transparent health services to the Indian Citizens. However, all the National Digital Health Mission activities are going to carry forward through the official portal of the National Digital Health Mission (NDHM). 

Digital Health ID, Digital Health Card Yojana

The NDHM Portal serves as a route map to the citizens to get enrolled for Digital ID Card, acts as a platform for the Doctors, Chemists/Pharmaceutical companies, and insurance companies to get register on the Health Card Yojana Portal. As a pilot project, the scheme has already been launched in Union Territories. In Chandigarh, soon after the announcement, a drive was conducted in which around 4000 citizens were registered for the Digital Health ID Card Scheme, and received their PM Modi Digital Health ID Cards immediately after the registration process. However, a few doctors were also registered under the NDHM Portal on the same day, which was a good start to this brainchild scheme of the government. The Digital Health Card Scheme is applicable in a few listed Union Territories, and the goal of the government is to expand it to all other states by 2022.

Who are eligible to apply for Digital Health ID Scheme?

The citizens or the doctors who are registering under the Digital Health Card Scheme have to fulfill the following eligibility criteria to avail the services of this scheme.

  • The objective of PM Modi Health Card Yojana is to make the scheme available for every Citizen of India.
  • Every Citizen who is a permanent resident of India can apply for the Digital Health ID Cards.
  • The Citizen must have an Aadhar Card to get register for the scheme.
  • The doctors who are willing to get registered under this scheme must possess a valid certificate from the Medical Association.
  • It is voluntary for the citizens to apply for the Digital Health ID Yojana.

Complete Details of National Digital Health ID Card Mission Scheme

Health ID:

  • Under the National Digital Health Mission Scheme, the citizens can apply directly to the NDHM portal in the first phase. 
  • Once after registering on the portal, the citizens can get a Unique Health ID Card similar to Aadhar Card. All the health-related information of the citizens is stored in the Digital Health ID Cards. Once applied, you can download health card within minutes.
  • All the medical details like prescriptions, patient history, medical diagnosis, and discharge summaries are stored in the unique health ID card. 
  • The doctors and the medical service providers will be allowed one-time access to the patient’s records during each consultation for the treatment.
  • However, the doctors and the medical service providers could access the data a single time that reduces the patients’ fear of confidentiality.
  • Under the National Digital Health Mission, the patients can access healthcare services remotely through tele-consultancies and e-pharmacies.

Digi Doctors:

  • The doctors can register as Digi Doctor by entering their details like Name, Email, Phone Number, Qualifications, Work Experience, Specializations, Registrations with Medical Councils, Year of Experience, etc.
  • Submission of Contact details of the doctors on the portal is voluntary, and it depends on the wish of the concerned Doctor.
  • To avoid fraudulent activities, the doctors have to provide authenticated documents like their registration in the medical council, their qualifications, etc.
  • However, the doctors can update their work experiences, skills, fellowships, etc. on the portal and map them to facilities they are associated with.

Health Facility Registry:

  • All the information related to the country’s public and private health facilities is stored and maintained in a single repository by the Health Facility Registry.
  • The health care services, both private and public, can add and update the details regarding the health care facilities, specialties, and hospitals’ infrastructure-related information periodically on the portal.
  • Through the health care registry, the concerned health care providers can sign documents digitally such as Patient Records, apply for empanelment, claims processing, etc. 

Personal Health Records (PHR)

  • Personal Health Records (PHRs) are digital records that contain all the health-related information of an individual that conforms to the standards of National Digital Health Mission.
  • All the data is being stored, managed, and controlled by an individual.
  • However, the Personal Health Registry (PHR) enables the individual to manage information about his/her healthcare.

Electronic Medical Records (EMR)

  • Electronic Medical Records (EMRs) is the repository of patient’s digital health-related information such as health history from a single hospital or a health care unit.
  • It can be termed as simple as the patient’s chart, where all the information related to the patient is stored in the digital format.
  • EMR’s plays a vital role in tracking the data from time to time.
  • Through EMR’s, the clinics can access the list of pending patients who are pending for medical check-up and diagnosis.
  • The doctors can check the patient’s response to the treatment by examining their Blood Pressure, Sugar Levels, and other parameters.
  • EMR’s allow monitoring and improving the quality of medical care to the patients.
  • EMR’s work on specific standards like Data Construct, Standard Medical Terminologies, and interoperability standards.

Required Documents for applying PM Modi Health Card Scheme

  • The core objective of the National Digital Health Mission is to reduce manual work and encourage paperless work.
  • The citizens no need to provide any hard copies of medical documents anywhere, except on the portal.
  • However, during the registration, the applicants have to enter Aadhar Card details for authentication and the few other details related to their health.

How to Apply for Digital Health ID card | PM Modi Health Card Yojana, Scheme Online Registration Form 2020-21

  • Click on the Create Health ID Option.
  • It takes the online user to the following page.
  • Click on the Create Health ID, and it redirects you to the following page.

  • Click on the tab Generate via Aadhar. You can see the following page, as shown below.

  • Enter the Aadhar Number, click on the checkbox agreeing to the terms and conditions and click on the Submit Button.
  • It then sends the OTP to your Mobile Number that is linked to the Aadhar.
  • Enter the OTP Generated to your Mobile Number in the given field.

  • Click on the Submit Button, after you enter the OTP.
  • It redirects you to the registration form to enter the basic details such as Name of the Applicant, Email ID, Mobile Number, and Address for Communication in the personal details section.

  • Later on, the applicant can upload his/her scanned medical documents on the portal.
  • After verifying all the details, click on the submit button.
  • After submission of the application, the citizens will receive their Unique Digital Health ID.

After the successful completion of the Application Form, the citizens can start Login to their Health ID Account.
